Who needs a DD Form 108?
The US Department of Defense created DD form 108, which is called the Application for Retired Pay Benefits, in order to review eligibility for retirement benefits. The form is to be used by members and former members of the Reserve Components to apply for retired pay at age 60.
What is DD 108 Form for?
Application for Retired Pay Benefits is a tool to enable the eligible member to provide all the necessary information to the US Armed Forces. Their duty is to do the following:

Identify the individual and his/her service record.

Determine eligibility for retired pay under 10 U.S.C. 1331.

Determine effective date that retired pay can and will commence.
Is Application for Retired Pay Benefits accompanied by other forms?

Where do I send DD Form 108?
The application is to be filed in one copy (or a duplicate for Naval personnel). The completed form must be addressed to the Armed Force in which the applicant is presently (or was last) a member. The addresses of possible destination are indicated in the text of form.
